For Physicians

In order to expedite the treatment of your patient please fax our office (281-240-4353) as many of the following as possible:

• Either our referral form, or your own
• Your patients demographics (with contact and insurance information)
• Your office notes indicating why we are seeing the patient, and what medications you have prescribed.
• Any diagnostics and imaging reports available, whether ordered by your office or others.  Please send the original films (or CD) with your patient for their appointment.  They will be sent home with the patient following treatment.

If your patient has an unusual condition, or you feel that our doctor needs to be aware of special circumstances about your patient or their pain, you are encouraged to call and speak personally to Dr. White.
